Please complete the embedded application form and follow the steps written below to submit your application. The embedded form includes personal and contact information, voluntary self-identification including race and identity, work authorization, exam results, and licensure, and two 250-word personal statements (one optional). Personal statement prompts are written below for review prior to beginning application form.

Personal Statement (250 words or less): Explain why you want to train at UPMC.
Optional Statement (250 words or less): Provide any additional information about life experiences or personal attributes not conveyed in the Personal Statement.
Submission of the application requires the following documents. All documents, aside from letters of recommendation, should be attached in the application form. Letters of recommendation should be emailed directly by recommender to specific fellowship program director. Fellowship director contact information is provided below.
- Updated curriculum vitae including current address, current and future training programs
- Official USMLE or COMLEX transcript (a PDF copy is acceptable)
- Three or more letters of recommendation, one of which must be from your residency program director (*As noted above, please have sent directly to fellowship program director)
- Medical School Transcript (a PDF copy is acceptable)
- ECFMG certificate, if an IMG
Please Note
The application, supporting documents, letters of recommendation, and interview must be completed by December 31 of the year prior to the training year that the candidate is applying for.
Completed applications will be independently reviewed by each fellowship program. After review, candidates may be contacted by the Fellowship. Interviews will include meeting with 2 fellowship directors from outside of the program. Interview reports are sent to the central fellowship office where they will be collated and the results sent to the individual fellowship director. If individual fellowship director decides to offer the candidate acceptance and funding is available an offer letter and contract will be issued.
